Student Residency Verification
A parent/guardian of all district students will complete one of the following student residency verification forms. Most families will use the first form (SRV1).
However, there are several additional forms that can be used for residents with special circumstances.

SRV1.pdf (Proof of residency from 2 categories)
SRV2.pdf (Letter of residence from Landlord in Lieu of Lease)
SRV3.pdf (Letter of residence to be used when the person seeing to enroll a student is living with a WCUSD#1 resident)
SRV4.pdf (Evidence of Non-Parent's Custody, Control and Responsibility of a student)
SRV5.pdf (Military Personnel Procedures)
