Physical Exam & Immunization Requirements for School Year 2018-2019

 

 

 

Physical Examinations and Immunization Requirements


 

Parents of students entering Pre-School, Kindergarten, Sixth, and Ninth Grades (or school for the first time) are reminded that student physical examinations and up-to-date immunizations MUST be completed PRIOR to October 15th.  Additionally, all middle school (jr. high) and high school students wishing to compete on any interscholastic athletic team are required to have an up-to-date physical exam on file.

 

Students entering Illinois schools from out-of-state must have up-to-date immunization records.  A physical examination is necessary if none comparable to Illinois requirements has been completed within one year prior to the date of entering an Illinois school.  Lead screening is a part of the physical for Pre-School and Kindergarten students.

 

Records of students transferring from other school districts within Illinois must also be checked for immunization compliance.  An additional physical examination is not required for grade levels other than Pre-K, Kindergarten, Sixth, and Ninth (or entering school for the first time) if compliance requirements have been met in that district.

 

Physical examinations must have been completed within one year to the date of entering school for Pre-K, Kindergarten, Sixth, and Ninth grades (or entering school for the first time) by a physician licensed to practice medicine in all its branches.  Sports Physicals must be done every year for sports, cheerleading, dance, etc. – students cannot practice or participate without it.  Students will be excluded from school if they do not provide proof of having received the vaccinations or physicals.  Vaccinations and physical exams are due by October 15th.

 

Dental exams are due by May of the current school year.  Vision exams for students entering Kindergarten are REQUIRED by October 15th.  After that date, reports cards are held until the exam or proof of an exam is shown.
