Consequences for Inappropriate Behavior
Students will receive the following consequences for inappropriate behavior:
Level 1 - Individual expectation reminder
Level 2 - Teacher detention and self-assessment form
Level 3 - Office referral
Level 4 - Parent conference

When Level 2 occurs, teachers are required to fill out a Teacher Discipline Report and students are required to fill out a self-assessment form. Below are examples of these forms by grade level:
